Successful Treatment of a Complex Case of Kidney Failure Linked to Severe Muscle Injury in Udaipur

Udaipur: In a rare case highlighting unexpected causes of kidney failure, doctors at Paras Health Udaipur successfully treated a 27-year-old man who was admitted with renal complications due to muscle injury. This case challenges the traditional notion that kidney failure is primarily caused by diseases like diabetes, hypertension, or glomerular diseases. Amit Yadav (name changed) was admitted to the multi-specialty hospital in Udaipur with alarming symptoms, including reduced urine output, bilateral pedal edema (swelling in the lower limbs), abdominal pain, and red-colored urine. These symptoms had persisted for over a week. Upon admission and further investigation, it was found that his creatinine and potassium levels were critically high, along with inflammation of the pancreas, liver injury, and kidney swelling. He also had multiple injuries on his body, allegedly from a physical altercation. Further tests revealed that the patient was suffering from rhabdomyolysis, caused by severe muscle breakdown. The presence of myoglobin in his urine was confirmed through diagnostic tests, and his creatine phosphokinase (CPK) level was over 50,000. All these factors led to acute kidney injury (AKI), resulting in reduced urine production.

Dr. Ashutosh Soni, Consultant Nephrologist at Paras Health Udaipur, said, "This case highlights the importance of recognizing unconventional causes of kidney failure. Amit’s kidney failure was caused by myoglobin, a protein released from damaged muscles, which accumulates in the kidneys and damages them. Early identification and immediate intervention are crucial for recovery in such cases." The patient was admitted to the Medical Intensive Care Unit (MICU), where he underwent multiple rounds of hemodialysis to control his kidney function. Supportive measures, including three units of packed red blood cells (PRBC) and metabolic stabilization, were also provided. Gradually, his urine output improved, and signs of kidney function recovery were observed. The treatment continued for 8 to 10 days, and after the initial stages, the patient's condition stabilized. After several weeks of continuous care and monitoring, he fully recovered and was discharged from the hospital in a stable condition.

This case underscores the importance of recognizing various causes of renal failure, including physical trauma and muscle injuries, and highlights the need for timely multidisciplinary medical care to manage complex cases effectively.
